How to Request Your Health Care Records
Current and former clients may request copies of their counseling records maintained by Thrive & Heal Therapy PLLC. Requests must be submitted using one of the following methods:

Client records are retained for a minimum of seven (7) years, or five (5) years after a former minor client reaches eighteen (18). Records may be requested through the client portal if needed. Administrative fees may apply.
If records are requested on behalf of another individual, documentation verifying legal authority is required, such as guardianship papers or a medical power of attorney. Identity verification may also be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.
Filing a Consumer Complaint
f you believe you have been impacted by fraudulent business practices in the State of Texas, you may contact the Office of the Attorney General to file a consumer complaint. Clients or members of the public may submit complaints through the Texas Office of the Attorney General’s Consumer Protection Division.

When submitting a complaint, please include relevant dates, names, and any supporting documentation, if available.
License Violation Concerns
Thrive & Heal Therapy PLLC is regulated by the Texas Behavioral Health Executive Council (BHEC), the licensing authority for Licensed Professional Counselors in the State of Texas; Counselor and Social Worker board of Ohio – Counselor, Social Worker, and Marriage and Family Therapist Board for Licensed Profssional Clinical Counselors in the state of Ohio; and Board of Behavioral Health Examiners as Licensed Professional Counselor in the state of Arizona.

If you believe a clinician has violated the applicable code of ethics or acted outside the scope of their licensure, you may contact the appropriate licensing board.
